Care Coordinators at A & D help older adults and their families determine what specific needs may be required for a person to stay in their own home safely. Care Coordinators help identify community based services available to meet those needs. We will work closely with each individual and their family to create a care plan that addresses each of those needs. Care coordination services are especially useful for those trying to take care of a loved one at a distance.
Years of experience in providing care for older adults at home has allowed us to develop relationships at all levels of the healthcare system in Connecticut. Our coordinators are knowledgeable about resources and all possible options available to meet your needs.
